1. Middle Names: I only entered my surname and first name on TixPlus, but my passport includes my middle name too. Do I need to add the middle name, or is first usually enough?
2. Capitalization: My name is in standard sentence case, but I've seen some people say it should be ALL CAPS to match the passport machine-readable zone. Does the casing actually matter?
3. Katakana: In the section "変更後のお名前(カタカナ)", I just wrote my name in romaji again. Should I write it in katakana, or is romaji enough?
