The health inspection world grades a score below 70 as failing, and one restaurant received a failing score from inspections conducted on Aug. 25–29.

Worst scores in this week’s health inspection

This week’s lowest health inspection score was a 62 earned by a Kyoto Japanese Cuisine, located at 202 South Belair Road, in Martinez.

During the inspection, the inspector noticed food being improperly reconditioned for usage, which resulted in a nine-point deduction.

“Observed multiple sources of food being reconditioned – Rice kept in room temp rice cooker overnight, turned back on to be served – Butter/margarine in preptop cooler kept overnight with food debris – zucchini dropped on floor that appeared to be put back in cooler,” the report stated.

The inspector also reported observing food being stored uncovered, resulting in four points docked from the health score. 

“Observed multiple areas; sushi cooler, [walk-in-cooler], preptop cooler; with food being stored uncovered,” the report stated. “Ensure ALL food is stored uncovered ALWAYS.”

Another four points were deducted from the health score for the inspector finding a lack of clean surfaces in the kitchen and dishes not being properly sanitized.

“Observed multiple food contact surfaces; grill area, fryer area; not properly cleaned. Dishes in 3-comp sink and dishwasher not properly sanitized,” the report stated. 

After losing another three points for observing foods stored on the floor of the dry storage area, the inspector deducted four more points for finding ready to eat food prepared and stored without dates on them.

“Observed several items that were ready to eat that should have dates on them. Example: cooled rice, white sauce,” the report stated. “Ensure all cooled, ready to eat foods are dated properly.”

The establishment lost three more points for employees not wearing proper food prepping gear in the kitchen.

“Observed multiple employees not wearing hair restraints,” the report stated. 

Throughout the inspection, in addition to losing points for single service area with food debris, the inspector docked another point for seing more unclean kitchen surfaces.

“Observed multiple areas, equipment with significant food buildup,” the report stated. “Ensure ALL equipment, nonfood contact surfaces are cleaned THOROUGHLY.”

The second lowest score of the week was a 77 given to Jerk City Truck, located at 2147 Eastside Court, in Augusta.

The inspector deducted four points for noticing a lack of food safety paperwork on site.

“Observed no Serv Safe Certification on truck,” the report stated. “Post up to date Serv Safe certificate.”

During the inspection, the inspector deducted another four points for finding the employee handwashing sink filled with items.

“Observed handsink filled with junk,” the report stated. “Keep handsink free for handwashing.”

The inspector also found no sanitizer on the food truck, which resulted in another four point reduction.

“Observed no commercial grade sanitizer in truck,” the report stated. “Truck MUST have sanitizer for cleaning food contact surfaces.”

For finding a can of pesticide on the truck, the inspector deducted another four points.

“Observed can of residential pesticide on truck,” the report stated. “Use only licensed food safe pest control.”

For not seeing any thermometers in any of the cold holding units and noticing unclean gaskets, the score was reduced by another four points.

After observing no permit or inspection report being displayed within the truck, the inspector took two more points off the establishment’s score.

“Observed no permit being displayed anywhere in truck,” the report stated. “Observed no inspection report being displayed anywhere in truck.”

Following the Jerk City Truck inspection, the third lowest score of the week was a 79 given to a Country Inn & Suites, located at 103 Sherwood Drive, in Augusta.

The inspector deducted four points for noticing a lack of proper employee paperwork.

“All employees must have signed employee illness reporting sheet on file and available for inspection,” the report stated.

During the inspection, the inspector deducted another nine points for food being improperly stored.

“Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit. (Waffle mix, items in self-serve reach in cooler),” the report stated.

Another nine points was docked from the establishment’s health inspection score due to the inspector finding a lack of written proper procedures for food safety.

“Write standard operation procedure for time as food safety control on [temperature control safety] food to be discarded at end of service period,” the report stated. “List all foods that time is used as control. Other foods must be held at 41F or below, 135F or above.”
